Consent to Treatment & Telehealth

1. Voluntary Consent to Treatment

By accepting this Consent to Treatment & Telehealth (“Consent”), you voluntarily request and authorize the licensed healthcare providers affiliated with Crimson Performance Healthcare Inc. (“Crimson,” “we,” “us,” or “our”) to evaluate you and to provide the medical care, treatment, and services you request through our platform.

Your consent is voluntary. You may ask questions before agreeing, and you may withhold or withdraw your consent at any time as described in Section 9. This Consent supplements, and does not replace, our Terms of Service and our Notice of Privacy Practices.

2. The Provider–Patient Relationship

Care is delivered by licensed healthcare providers using telehealth — the use of electronic communications to enable a provider to evaluate, diagnose, and treat you remotely. A provider–patient relationship is established when a Crimson provider reviews your information and agrees to provide care. Whether any particular treatment, medication, or service is appropriate for you is determined solely by your provider, in their independent clinical judgment.

Crimson’s technology platform coordinates and supports your care; it does not itself practice medicine or make clinical decisions.

3. Nature, Benefits, and Limitations of Telehealth

Telehealth may offer benefits including improved access to care and greater convenience. It also has limitations. Your provider will not perform an in-person physical examination, and care is based on the information you provide and on any laboratory or diagnostic results available. You understand and accept that:

  • Information you provide that is inaccurate or incomplete may affect your provider’s ability to deliver appropriate care.
  • In rare cases, a condition may not be diagnosed or treated as effectively as it would be through an in-person visit, and your provider may determine that telehealth is not appropriate and that you should be seen in person.
  • Technical failures (for example, equipment or connectivity problems) may interrupt or delay care.
  • Telehealth may not be appropriate for every condition or every patient.

4. Treatment and Prescriptions

Any prescription, medication, dose, or course of treatment is provided at the sole discretion of your treating provider, based on their clinical judgment and applicable law. Submitting an intake, completing a questionnaire, or making a payment does not guarantee that you will be prescribed any particular medication or receive any particular treatment.

If your provider determines that a requested treatment is not clinically appropriate for you, it will not be prescribed.

5. Laboratory Testing and Release of Results

Your care may involve laboratory testing ordered by your provider or arranged through Crimson. You authorize the performing laboratory to release your completed test results to Crimson and to your treating provider so that they may be used in your care and made available to you through our platform.

You understand and acknowledge that:

  • Your laboratory results may be made available to you in your patient portal, in some cases before your provider has reviewed them or discussed them with you.
  • Laboratory results displayed to you are provided for your information and do not, by themselves, constitute medical advice or a diagnosis specific to your condition.
  • Questions about the meaning of your results should be directed to your Crimson care team, and reviewing your results on your own is not a substitute for communication with your provider.
  • An abnormal or out-of-range result does not necessarily indicate a serious condition, and a normal result does not rule one out.

6. Risks and No Guarantee of Outcomes

As with any medical care, treatment carries risks, and results vary from person to person. No provider can guarantee a particular outcome. Your provider will discuss the material risks, benefits, and alternatives relevant to any treatment they recommend.

7. Your Responsibilities

You agree to provide accurate, current, and complete information about your health, medications, and medical history; to follow the instructions and care plan provided by your provider; and to seek in-person or emergency care when your condition warrants it. You agree to keep your contact information up to date so that we and your care team can reach you.

8. Not for Medical Emergencies

The Services are not intended for use in medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Do not use the platform to communicate time-sensitive or emergency medical needs.

9. Right to Withhold or Withdraw Consent

Your consent is voluntary, and you have the right to withhold or withdraw it at any time without affecting your right to future care. Withdrawing consent will not affect care already provided, and some withdrawals may end your ability to continue receiving treatment through the platform. To withdraw consent, contact your care team or support@crimson.healthcare.

10. Alternatives to Treatment

Alternatives to telehealth treatment through Crimson include seeking care in person from your own physician or another healthcare provider. You may choose to pursue these alternatives at any time.

11. Acknowledgment

By indicating your agreement during intake, you confirm that you have read and understand this Consent, that you have had the opportunity to ask questions, and that you voluntarily consent to evaluation and treatment through Crimson’s telehealth platform.
